I am a private-pay only practice and not in-network with any insurance providers. You are responsible for 100% of the fee at the time of service. I accept credit card, HSA, and FSA as a form of payment.

Some insurance plans allow for out-of-network benefits (OON). If this is something you’re curious about, I’d encourage you to contact your insurance provider to see if you are eligible for any OON benefits for mental health providers. Make sure to inquire what these benefits look like for your specific plan so that you’re fully informed.

I can then provide a form called a “Superbill” (service receipt) upon request. You would then submit this form to your insurance company for potential reimbursement. This reimbursement is not guaranteed, so please understand this prior to making treatment decisions. It is your responsibility to determine whether you have OON benefits with your insurance provider. Beyond the Labyrinth has no interaction or communication with insurance companies besides providing a superbill for our clients. Please also be aware that your insurance company will require a diagnosis for Superbill submission, which will then become a part of your permanent medical record.
